FLSA Status: Non -Exempt Reports to: Managing Director/Compliance

Job Summary:

The Care Coordinator will help manage a client's condition and assist in their treatment plan to

include tasks related to crisis intervention, case management, client advocacy, referrals and

prevention. The Care Coordinator will monitor and coordinate treatment plans with clients,

educate and connect them with various clinical professionals as needed. Additionally, this

position will oversee the monitoring of all screening and enrollment procedures, track progress,

and report program outcomes to the relevant individuals or department.

Essential Functions:

? Assist clients in gaining access to needed medical, social, educational and other services.

? Assist in the application process for services with new clients.

? Works collaboratively with clinicians in the CCRS process.

? Refers clients to community resources for housing or treatment to assist in recovery

from mental or physical illness following through to ensure service efficacy.

? Discharges planning evaluation will be made in a timely manner to ensure that

appropriate arrangements for post-discharge care will be made to avoid unnecessary

delays in discharge.

? Conducts self in professional manner.

? Maintains professional relationships with other departments, external organizations,

service providers, and families of patients.

? Responsible for data input and tracking related enrollment and screenings for programs

such as S.O.R., State Funding, and the Virtual Walk-In Clinic, and other programs as

directed.

? Responsible for tracking and updating client outcomes on a monthly basis.

? Ensures that all data is collected on time and entered accurately.

? Updates and maintains databases and spreadsheets.

? Reports and presents findings based on data entered/tracked.

? Adheres to any pertinent laws, regulations, and ethical standards related to data

collection and client care.

? Performs other duties as assigned.

? Completes all required Oxbow Center training courses.

Education and Experience:

- Bachelor's degree in sociology, psychology, or related field.

- Minimum 2 years' experience in a healthcare setting
